BULATS Online Account Application Form

Dear School, Dear Customer

We kindly ask you to fill in this BULATS Online application form in order to get your online account. Once we set up your account, you will receive an e-mail from the BULATS online system that will allow you to log into the system and set up your own password. As you are responsible for all orders, please make sure to keep your secret password in a safe location.

NOTE: There are two types of entities that you will be working with: The Indirect Customer and the Test Administrator(s). Each Indirect Customer has to name at least one Test Administrator. This can but does not have to be the same person. If the Indirect Customer has the same e-mail address (user name) AND the same password for the Test Admin role, Online BULATS will prompt the user to select which of the two roles he/she wishes to login as.
